Very good condition. Federation logo is embroidered. Puma logos are flock and embossed with minimal wear. Correct sized player issue front/rear player number are flock and heat applied. Lower shirt motif and sleeve flags are PolyFlex and heat applied with cracking and wear. Few small pulls and bobbles across shirt. Colours and stitching in excellent condition.

Worn as the Czech Republic made it to the semi-finals of Euro 2004 before being eliminated by a silver goal by the eventual winners. This would be the only major competitive match to be decided by a silver goal as it both methods would be removed from the Laws of the Game after the tournament ended.
